All student-athletes MUST provide the following:

• A sports physical must be submitted on the ECC official physical form. This is the ONLY form that will be accepted as proof of physical. 
• Completed medical health questionnaire 
• Completed student-athlete insurance questionnaire
• Copy of insurance card (front & back)
